However, its male equivalent, andropause or age-related androgen deficiency (ARAD), remains largely unknown. Unlike the female transition, andropause does not mark a sudden end to fertility, but rather a gradual decline in testosterone levels, which can profoundly affect men&#8217;s vitality and metabolic health from their forties onwards.<\/p><h2>\u00a0<\/h2><h2 class=\"p1\">1. What is andropause?<\/h2><p class=\"p2\">Andropause corresponds to the decrease in bioavailable testosterone levels associated with aging.<\/p><ul><li><p class=\"p2\"><strong>Hormonal decline: <\/strong><span class=\"s1\">From the age of 40 onwards, total <\/span>testosterone levels decline by an average of 1% per year, while free testosterone (the biologically active fraction) declines by 1.2% per year.<\/p><\/li><li><p class=\"p2\"><strong>A biological reality: <\/strong><span class=\"s1\">According to the European Male <\/span>Aging Study (EMAS), approximately 2.1% of men aged 40 to 79 have symptomatic late-onset hypogonadism, but this rate increases significantly in men with comorbidities such as obesity or diabetes.<\/p><\/li><\/ul><h2>\u00a0<\/h2><h2 class=\"p1\">2. The \u201ctherapeutic gap\u201d: telling figures<\/h2><p class=\"p2\">Beyond simple clinical observations, research shows a major discrepancy between the prevalence of androgen deficiency and its actual treatment. Andropause remains one of the neglected areas of preventive medicine for men.<\/p><p class=\"p2\">The BACH (Boston Area Community Health Survey) study, published in the Journal of Clinical Endocrinology &amp; Metabolism, found that while approximately 24% of men have testosterone deficiency (total or free), only 5% to 10% of those with clear clinical symptoms actually receive treatment.<\/p><p class=\"p3\">This \u201ctreatment gap\u201d can be explained by the insidious nature of the symptoms, which are often confused with stress or the simple effects of aging, but also by a persistent taboo that prevents men from talking to their doctors about it.<\/p><h2>\u00a0<\/h2><h2>3. Symptoms: knowing how to identify them<\/h2><p class=\"p2\">Androgen deficiency manifests itself through a set of physical and psychological signs that develop slowly:<\/p><p>\u00a0<\/p><h4 class=\"p2\">The sexual and intimate sphere<\/h4><ul><li>Decreased libido (sexual desire)<\/li><li><p class=\"p2\">Erectile dysfunction and reduced frequency of mor<span class=\"s1\">ning erections. <\/span><\/p><\/li><\/ul><h3>\u00a0<\/h3><h4 class=\"p2\">The physical and metabolic sphere<\/h4><ul><li><strong>Sarcopenia:<\/strong> Loss of muscle mass and decrease in phy<span class=\"s1\">sical strength<\/span><\/li><li class=\"p2\"><strong>Adiposity:<\/strong> Increase in fat mass, particularly in the ab<span class=\"s1\">dominal area <\/span><\/li><li class=\"p2\"><strong>Fatigue:<\/strong> Persistent lack of energy and sleep distur<span class=\"s1\">bances<\/span><\/li><li class=\"p2\"><strong>Bone density:<\/strong> Increased risk of male osteoporosis<\/li><\/ul><h4>\u00a0<\/h4><h4 class=\"p2\">The psychological sphere<\/h4><ul><li class=\"p2\">Irritability, decreased motivation, and mood disorders: <span class=\"s1\">anxiety and depression<\/span>.<\/li><li class=\"p2\">Difficulty concentrating and feeling of \u201cbrain fog.\u201d<\/li><\/ul><h2>\u00a0<\/h2><h2 class=\"p1\"><span class=\"s1\">4.<\/span>\u00a0Long-term consequences and cardiovascular risks<\/h2><p class=\"p2\">Testosterone plays a major protective role in metabolism. A prolonged deficiency is not just a matter of comfort; it is a systemic health issue that promotes insulin resistance and alters the lipid profile, thereby increasing cardiovascular risk (atherosclerosis).<\/p><p class=\"p2\">It is estimated that 20% to 30% of cases of male osteoporosis are directly attributable to androgen deficiency.<\/p><p>\u00a0<\/p><h2 class=\"p1\">5. Substitution treatment (ST): a rigorous approach<\/h2><p class=\"p2\">This medical therapy requires a precise approach. It is intended for men whose deficiency has been confirmed by repeated blood tests and debilitating clinical symptoms.\u00a0<\/p><p class=\"p2\"><strong>The benefits of tailored care:<\/strong><\/p><ul><li class=\"p2\">Restoration of libido and sexual function<\/li><li>Improved body composition (more muscle, less fat)<\/li><li class=\"p2\">Protection of bone capital<\/li><li class=\"p2\">Significant improvement in mood and cognitive abili<span class=\"s1\">ties<\/span><\/li><\/ul><p>\u00a0<\/p><p class=\"p2\">Treatment (in the form of gels, injections, or patches) requires close medical monitoring, including PSA (prostate) and hematocrit monitoring, to ensure optimal safety.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t","protected":false},"excerpt":{"rendered":"<p>Menopause in women is a well-recognized public health issue.
